Baptism Preparation

Parents who desire to have their child(ren) baptized must submit the following: 

• Baptism Form

• Copy of the child's birth certificate 

• Godparent Certification Form from the Godparent's respective parish with parish seal

The Baptism Form and Permission Form (if the Godparent is a parishioner of Our Lady of Purification) are provided on this page. Kindly download, complete, and submit to our office via email or in person. Additionally, we require the parents and godparents to attend a Baptism Preparation Class before the Baptism.

Godparent Certification Form

The individual requesting to be a Godparent must have received the sacrament of Confirmation.  If the marital status is single, he or she must not be living with an unwed partner. Please review the form for further requirements in being a godparent. The individual must complete the Godparent Permission Form and allow ample time for the Pastor / Rector / Parochial Vicar to acknowledge and approve or deny their request.

All forms must be submitted before a Baptism Preparation Class and Baptism Ceremony can be scheduled. 

Baptism Preparation for Adults

Non-Catholic adults who wish to learn more about Baptism in the Catholic Church are invited to participate in the Rite of Christian Initiation for Adults (RCIA) program. Please see the Religious Education tab above.
